Start with the people supported, regulated activities, staff roles, location and day-to-day decisions. For each policy, name the owner, approval date and review trigger. Add local safeguarding contacts, reporting and escalation routes, medicines arrangements where relevant, out-of-hours cover, record system and who checks implementation. Ask a worker to explain a realistic situation and compare that with the text. Include the views of people receiving care and staff. Record what changed after review.
