Choose a manageable area of practice and compare the written record with what people experience. Record evidence that supports the finding and information that is still missing. A useful review ends with a named action, a realistic completion date and a way to check that the action helped.

Keep original records alongside your working notes, give the next action an owner and agree how progress will be checked. Reuse the process when the situation changes, while retaining earlier versions so the sequence remains clear.
