CQC Evidence Folder: How Do You Show Care Is Working in Practice?

Start with the question the evidence should answer

A folder of policies is not proof that people receive safe, effective and person-centred care. CQC's assessment framework asks five key questions: is the service safe, effective, caring, responsive and well-led? For each issue, show the practice, the record, what people experienced and what changed after a concern. Use the current CQC framework for the service being assessed, because assessment approaches can change.

What belongs in a useful evidence trail?

Choose a small sample across people, workers and shifts. Link a current care plan and risk assessment to visit notes or observations; staff role and competence records to work allocated; medicines support to appropriate records and review; and incidents or complaints to investigation, action and later checks. Keep an index pointing to the secure source record, with an owner, review date and status. Do not copy confidential files into a widely shared folder simply to make it look full.

How can a manager test it?

Pick one real concern and trace it both ways. Can you see what staff were instructed to do, what happened, how the person experienced it, and whether a manager checked that the change worked? Mark missing, expired or contradictory evidence as a live action with a responsible person and deadline. At the next governance meeting, review whether the action improved the person's experience, not just whether a form was completed.

What does Regulation 17 add?

CQC's Regulation 17 guidance addresses effective governance, including assurance and auditing systems and accurate, complete, secure records. An evidence folder can help retrieve those records, but the provider must maintain the underlying systems and act on risks. A template cannot establish compliance by itself.
