Free practical resource
A practical three-page starting point for families and SEND supporters in England who want to compare the special educational provision in a final Education, Health and Care Plan with what is actually being delivered. The aim is a clear, factual conversation about support: what the plan says, what is known, what remains uncertain and what response is needed.
What is included
The check includes a final-plan baseline, a provision check, a short delivery record, a worked fictional example, a message framework and links to official sources. It helps you distinguish an unconfirmed concern from a confirmed gap, record the source of your information and identify the person or service you need to ask. It does not ask you to interpret a child's diagnosis or make a legal determination.
How to use it
Keep the current final plan beside you and select one provision to check first. Copy its wording accurately, note the relevant period and record evidence from school, the young person or another reliable source. Where a session or arrangement has not been confirmed, mark it as uncertain and ask a focused question. Use the message framework to request clarification or a practical response, then date the reply and any agreed action.
Files and access
You receive a editable Microsoft Word document, the same PDF for reading or printing, and a short START-HERE guide in a ZIP file. The PDF is not a fillable form. You will need suitable software to open Word and PDF files and extract a ZIP archive. No subscription or separate consultation is included.

Scope, version and permission
This is an independent preparation and record-keeping aid for England. It explains the general duty in section 42 of the Children and Families Act 2014, including the exception where a parent or young person has made suitable alternative arrangements.
