It turns scattered notes, emails and recollections into a dated record that can support a focused discussion with the school, provider or responsible authority.
What is included
The workbook contains a final-plan baseline, a delivery log with clear status choices and a fictional example, an evidence index, a page for the child or young person's views, draft requests for both uncertain and confirmed gaps, meeting questions and a follow-up action record. How to use it
Start with one provision and a manageable review period. Keep the wording of the final plan separate from what somebody reports happened. Record the source and date of each observation, keep original correspondence, and identify missing information before drawing conclusions. Use the meeting questions to agree who will act, by when and how delivery will be checked. Revisit the action record after the agreed date rather than treating a meeting itself as the outcome.
