CQC Safe Key Question: Evidence Checklist for Care Providers
Updated: Oct 11
The CQC Safe key question asks whether people are protected from abuse and avoidable harm. This article was checked against current CQC guidance in August 2026 and is written for adult social care providers in England.
Relevant legal and regulatory standards
The Safe key question connects to several fundamental standards, including:
Providers should use CQC's current assessment framework and guidance relevant to their regulated activities.
Detailed Safe evidence checklist
Safeguarding
Keep concerns, immediate protection, consent or best-interest decisions, referrals, police contact, outcomes, learning and action. Test whether staff know local reporting routes and how to escalate outside management.
Risk and care planning
Risk assessments should be person-specific, proportionate and connected to care plans, daily records and reviews. Record the person's views and how positive risk-taking is supported.
Medicines
Evidence should include current instructions, MAR records, PRN protocols, competency, audits, errors, clinical advice, learning and follow-up. The Medication Competency & Error Review Pack supports structured recording.
Safer recruitment and staffing
Check identity, work history, references, right to work, role-appropriate DBS evidence, qualifications and risk decisions. Staffing records should demonstrate enough suitably skilled people, realistic travel time, safe allocation and contingency cover.
Incidents and learning
Record facts, harm, immediate action, reporting, investigation, duty of candour, action and effectiveness. The £14.99 Incident Reporting, Learning & Duty of Candour training supports this process.
Environment, equipment and infection control
Keep checks, maintenance, risk assessments, staff competency, infection-control audits, contingency arrangements and actions.
Personal insight: follow the complete care journey
My compliance experience taught me to sample one person's journey across every connected record: assessment, care plan, risk, rota, visit log, care notes, medicines, incidents and review. Then sample the assigned workers' recruitment, training, supervision and competence. This reveals contradictions that separate folder checks can miss.
The Staff & Client Compliance Checklists Pack is designed around these repeatable checks. Employers can use the Training Matrix & Staff Compliance Tracker alongside the £14.99 Safeguarding Adults Essentials training.
