What Are CQC Quality Statements and How Should Providers Evidence Them?

CQC quality statements are the commitments used to assess health and adult social care services in England. This guide was checked against CQC's published assessment framework in August 2026.

The 2026 CQC structure

CQC still assesses services through five key questions: Safe, Effective, Caring, Responsive and Well-Led. Under those questions sit quality statements. The wording matters because a provider's evidence should demonstrate what each statement looks like in practice.

Use the official CQC assessment framework as the controlling source. Looking Glass Care material helps organise evidence but does not replace it.

The six evidence categories CQC may use

CQC groups evidence into six categories:

  1. People's experience of health and care services — what people and representatives say about safety, respect, choice, access and outcomes.

  2. Feedback from staff and leaders — what workers, managers and leaders report about practice, culture, resources and risks.

  3. Feedback from partners — information from local authorities, commissioners, health professionals, safeguarding partners and other organisations.

  4. Observation — what is seen during visits or other assessment activity.

  5. Processes — policies, care records, audits, recruitment, medicines, complaints, incidents, training and governance systems.

  6. Outcomes — what changes for people, including safety, independence, wellbeing and avoidable harm.

A policy alone is process evidence. It becomes stronger when staff understand it, records show it was followed, people describe a positive experience and audits confirm an outcome.

A practical quality-statement evidence log

For each quality statement, record:

  • the evidence source and exact location;

  • the person responsible for it;

  • the period or sample covered;

  • what the evidence shows;

  • gaps, contradictions or risks;

  • action owner and completion date;

  • how effectiveness will be checked;

  • the next review date.

Personal insight from compliance work

Across more than 15 years of varied care experience, including hands-on compliance and management work, I learned that the biggest weakness is often disconnection. A training tracker may say “complete” while supervision identifies uncertainty; a policy may be current while care notes tell a different story; or an action may be closed without anyone checking whether practice improved.

That is why Looking Glass Care focuses on connected evidence rather than paperwork for appearance's sake. The system should help the provider see real practice before a regulator has to point it out.

Training for managers and teams

The £14.99 Manager Compliance Tracking and Monitoring training helps managers organise monitoring, ownership and follow-up. Employers can combine this with the Training Matrix & Staff Compliance Tracker to monitor learning and evidence across staff.

When to seek support

Looking Glass Care can help with evidence mapping, compliance-system review, audit planning, policies and practical improvement plans. Review services for organisations or arrange a free consultation to identify the most relevant service, product or training combination.

This is independent practical support, not legal advice, CQC representation or a guaranteed assessment outcome.
