Checklist
Safe: check one recent incident, medicines or safeguarding concern and its follow-up.
Effective: check a care need, consent decision and relevant staff competence in practice.
Caring: hear from a person or family member in an accessible way.
Responsive: check that a changed preference reached the care plan and staff.
Well-led: check one audit gap was closed and the result retested.
Next step
Choose the highest risk gap, give the action an owner and date, then check its effect. Escalate an immediate safety concern through normal procedures. CQC is testing changes to its assessment approach in 2026, so check its live guidance before using a fixed quality statement map.
