What Should Be in a Care Worker Personnel File?
Updated: Oct 11
A care worker personnel file should demonstrate safe recruitment, suitability, induction, competence and ongoing support. It must also be accurate, secure and limited to information the organisation is entitled to hold.
From application to appointment
My experience building and checking compliance trackers showed that a green spreadsheet cell is only useful when it links to reliable evidence. The file should make the recruitment decision understandable from beginning to end.
CQC Regulation 19 sets requirements around fit and proper persons employed.
Recruitment records
Application and full employment history, including explanations for relevant gaps.
Interview and values-based assessment records.
Verified identity and right-to-work evidence.
References and verification notes.
DBS eligibility, application, result and decision records.
Qualifications, professional registration and driving evidence where required.
Health information necessary to assess role capability and reasonable adjustments.
Risk assessments and conditions attached to an appointment.
Offer, terms, role description and signed employment documents.
Ongoing employment records
Keep induction, Care Certificate progress where applicable, role-based training, practical competency, supervision, appraisal, probation, performance support and development records. Include current role restrictions and agreed reasonable adjustments without spreading unnecessary confidential medical information.
Record complaints, concerns or disciplinary matters fairly, with investigation, employee response, outcome, appeal and retention decisions. Separate allegation from finding.
Audit questions
Can you verify the original evidence? Does the role match the check level? Are dates and names consistent? Is anything expired? Are restrictions visible to people who allocate work? Has competence been observed? Is information stored securely and retained appropriately?
