Practical use and review
For people turning information into practical action
Choose a manageable area of practice and compare the written record with what people experience. Record evidence that supports the finding and information that is still missing. A useful review ends with a named action, a realistic completion date and a way to check that the action helped.
Put People's Experience and You Said We Did Pack to work
Use a small sample, keep the reasoning transparent, involve the relevant people and return to the same issue after the change.
Start with one real question and complete only the sections relevant to it. Date each entry and distinguish evidence from opinion. Keep original records alongside your working notes, give the next action an owner and agree how progress will be checked. Reuse the process when the situation changes, while retaining earlier versions so the sequence remains clear.
Make the result useful
Keep the finished record clear enough for another person to understand without a separate explanation. State what you checked, the period covered, the evidence available and the unresolved question. Avoid filling every box simply to make the document look complete. Mark a point as not applicable or not yet confirmed where that is more accurate, and record what you will do to resolve it.
Scope and current guidance
Use the resource in the setting and jurisdiction described above. For use in England, check current official guidance and any relevant local procedure before relying on a requirement. The resource supports preparation and record keeping; it does not guarantee a regulatory rating, registration, legal outcome or a particular decision.
