EHCP annual review checklist for parents in England
Updated: Oct 11
An annual review is a chance to check whether an education, health and care plan still describes the child or young person accurately and whether the specified support is actually happening. Take the latest final plan and recent evidence into the meeting.
Would you like help preparing for your review? Book one hour of EHCP and SEND support for £35, with preparation of agreed documents included. Prefer to organise the information yourself? See the editable Annual Review and Placement Pack. You can also start with a free 30-minute consultation without committing to a purchase.
Checklist
Write down the review date, school contact and local authority case officer.
Read sections B, E, F and I of the final plan. Mark outdated needs and unclear provision.
Ask for school and professional reports; list each item of evidence with its date.
Invite the child or young person to share views in the way they prefer.
Describe progress, unmet needs and the effect of missing support on everyday education.
List the exact changes you want to needs, provision, outcomes and placement.
If comparing settings, ask how each could deliver every relevant part of section F.
Send your contribution in advance, keep a copy and ask for confirmation of receipt.
At the meeting, record actions, owners, missing evidence and deadlines.
Check the local authority's decision within four weeks of the meeting and respond promptly to any proposed amended draft.
Next step
If placement or transport is urgent, tell the local authority about the safety or attendance issue now rather than waiting for the review.
